1. Part Number

Required

Fill the number of FAI part or assembly. It usually appears on the drawing, purchase document, BOM etc. We can put manufacturer's part number here if there is no purchaser part number.

Some companies will only specify drawing number and use it as de facto part number. In this case, it is suggested to put drawing number here and ask customer for approval

2. Part Name

Required

Name of the FAI part/assembly. It can appear as “Title” or “Description” in the drawing's title block, “Item Name” or “Description” in the BOM. For example, we will as “Electronics Cover” as part name for below item. Drawing credit: Aleph Objects, Inc. Reused under CC BY-SA 4.0 license.

3. Serial Number

Conditionally Required

Record the serial number of the FAI part. It should be a unique identifier to the part or assembly.

Put “N/A” if piece is un-serialized.

4. FAIR Number

Conditionally Required

Though AS9102 Rev.B put this cell as conditionally required, in best practice it is better to always fill this cell.

This number should be a unique identifier that names this FAIR (3 forms). It can be an internal report number, or document ID if you are using a document management system, or other format, as long as it is unique.

It may be referred in parent assembly's FAIR's Form 1 Column 18.

For example, we can use this structure: FAIR + PN + Revision + Date, substituting space with “_” (underscore)

5. Part Revision Level

Conditionally Required

This is the revision of the part.

Revision of part can be same to drawing revision, but not necessarily.

6. Drawing Number

Conditionally Required

Drawing number of the FAI part.

7. Drawing Revision Level

Conditionally Required

This is the revision of the drawing.

As best practice, when there is no revision, put No Change, N/A or “-” here, instead of leaving the cell empty. A blank cell will raise readers and reviewers' concern if it is forgotten.

8. Additional Changes

Conditionally Required

This cell is for any changes incorporated in this FAI part, that are not reflected in drawing revision or part revision. For example, manufacturing process change, customer ECO, limited design variations, etc. can be marked here.

9. Manufacturing Process Reference

Required

The reference in this cell is used to provide tractability to the specific manufacturing lot. If this first article part is picked from a manufacturing lot, you can record manufacturing lot number here. If this is the only part made in this production run, you can record Production Flow Diagram number, or Control Plan number, together with date code.

10. Organization Name

Required

Name of the organization performing the FAI. It can be your company, or a contracting inspection facility.

11. Supplier Code

Optional

The identification number of your organization provided by your customer. It usually appears in your customer's purchase order.

12. P.O. Number

Optional

Your customer's purchase order number.

13. Part/Assembly

Required

Put “X” where appropriate.

If this is an assembly FAI, you will need to fill Column 15 to Column 18, and submit the detailed FAI for each component.

14. Full/Partial

Required

Put “X” where appropriate.

If this is a full FAI, you don't have to give explanations to below questions.

Sometimes, partial FAI is also called Delta FAI. It is used when this FAI is only partially different from another one. For example, it is a new revision with partial change, is one of multiple configurations, or is with partial manufacturing changes. Please state the reasons in the cell (not numbered) underneath Cell 14, and record the part number and revision of the baseline part.

15. Part Number

Required when Cell 13 indicates “Assembly FAI”

Part number of lower level items of the assembly. This can be individual parts, or sub-assemblies.

16. Part Name

Required when Cell 13 indicates “Assembly FAI”

Name of the part in Column 14. Requirements are the same as Cell 2.

17. Part Serial Number

Required when Cell 13 indicates “Assembly FAI”

Serial Number of the part in Column 14. Requirements are the same as Cell 3.

18. FAIR Number

Required when Cell 13 indicates “Assembly FAI”

FAIR Number of the part in Column 14. Requirements are the same as Cell 4.

19. Signature

Required

Name or ID of the person who completed this FAIR.

AS9102B states that electronic identification or signature are also accepted.

Check “FAI Complete” if there are no nonconforming, or “FAI Not Complete” otherwise. Note that nonconforming need to be documented according to AS9102 4.4.

20. Date

Required

Date when Cell 19 is signed.

21. Reviewed By

Optional

Put printed name followed by signature of the person from your organization who reviewed and approved this FAIR, before sending to your customer. Per AS9102, electronic identification or signature are both acceptable.

It is not required if the FAIR does not need additional review after Cell 19 is signed.

22. Date

Optional

Date when Cell 21 is signed.

23. Customer Approval

Optional

This cell is used by your customer to record approval/rejection.

24. Date

Date when Cell 23 is signed.
